Adoption Forms and Filing

The following pleadings must be filed on most adoptions filed in Clay County.

Form 17: Family Court Information Sheet
Affidavit of expense
Motion for Guardian Ad Litem
Order for Guardian Ad Litem
Motion for Home Study
Order for Home Study
Consent of Natural Parent(s)
Consent of juvenile(s) if over the age of 14 or will turn 14 during the adoption

Filing

You must file three (3) extra copies of each pleading with your original.

Final Hearing

At the Final Hearing, one (1) copy of Certificate of Decree of Adoption and $30 per child payable to Missouri Department of Health is needed.
